PURPOSE: A three dimensional tooth form sample is provided to control the generation of a bio-film on an actual tooth, and to regenerate the condition of dental caries. CONSTITUTION: A three dimensional tooth form sample comprises more than one shape selected from the group consisting of the occlusal surface(110) of teeth, fissure, gingival(140), tooth surface pit(115), gingival crevice(130), buccal grooves(160), and cavities of teeth with dental caries. The three dimensional tooth form sample is provided with porcelain, a dental restorative material, or acrylonitrile butadiene styrene.
