Provided are: an optical fiber anchoring substrate (2) having a light-emitting surface (21) that is on the same surface as a light-emitting end surface (1a) of an optical fiber (1), and adhesion region surfaces (22); a collimator lens (4) positioned on the light-emitting end surface (1a) side of the optical fiber (1); and a lens holding member (3) that holds the collimator lens (4), and keeps the light-emitting end surface (1a) of the optical fiber (1) and the collimator lens (4) at a prescribed distance. The lens holding member (3) is fixed by adhesion to the adhesion region surfaces (22). This configuration provides an optical waveguide collimator and an optical switch device in which a member that holds a collimator lens does not detach easily.
