<P>PROBLEM TO BE SOLVED: To provide an imaging technique that can perform an image correction and is high in document reproducibility without being affected by a way of placing a document and a camera position even when a blank form has shades and fractures in corners. <P>SOLUTION: A positioning symbol is printed in a document sheet when imaging of the document is performed by a digital camera or a mobile phone with a camera such as a smart phone. By performing an image processing with this positioning symbol as a reference, an image correction (trapezoid correction) can be performed even when a blank form has shades and fractures in corners.
