An internal combustion engine comprising at least one cylinder module is disclosed. The cylinder module comprises a shaft (6), two pair of cylinders and a piston (1a, 1b) in each cylinder. The shaft (6) includes a first multilobate cam (5a) and a second multilobate cam (5b). The first multilobate cam (5a) is axially fixed to the shaft (6). The second multilobate cam (5b) is differentially geared to the first multilobate cam (5a) for axial counter rotation about the shaft (6). The two pairs of cylinders are associated with the multilobate cams (5a and 5b). The cylinders of each pair are diametrically opposed with respect to the shaft (6) with the multilobate cams (5a and 5b) inter-imposed in between. The piston (1a and 1b) in each cylinder is rigidly interconnected. Each multilobate cam (5a and 5b) comprises 3+n lobes, where n is zero or an even number integer. The axes of the pairs of cylinders are at an angle to each other of half of the number obtained by dividing 360 degrees by the number of lobes on one of the cam (5a and 5b). The reciprocating motion of the pistons (1a and 1b) in the cylinders imparts rotary motion to the shaft (6) via contact between the pistons (1a and 1b) and the camming surfaces of the multilobate cams (5a and 5b).
