摘要 <P>PROBLEM TO BE SOLVED: To configure a compact wiring structure of an actuator when a plurality of wiring boards are connected to one actuator. <P>SOLUTION: In a plurality of COFs 50, each one end portion of a base material, having a formed output terminal 53, is arranged in a prescribed direction along one surface of a piezoelectric actuator 7 and is extracted from the piezoelectric actuator 7 in parallel to the one surface, and is folded to the opposite side to the one surface. Further, the extraction directions of COFs 50, which are adjacent in the prescribed direction, from the piezoelectric actuator 7 are opposite to each other. Drive ICs 52 for the plurality of COFs 50 are arrayed in alignment in positions opposite to the one surface of the piezoelectric actuator 7 with a distance.
