<P>PROBLEM TO BE SOLVED: To provide a medical treatment instrument achieving both of the stability of a treatment operation when repeatedly accessing an internal organ under treatment through a port and various accesses to various internal organs. <P>SOLUTION: The medical treatment instrument 10 includes a retractor body 30 and a converter 20. The retractor body 30 is in a cylindrical shape and is indwelt in an incised wound. The converter 20 is freely detachably mounted on the opening end part 32 of the retractor body 30 and closes the opening end part 32. Also, on a plurality of parts of the converter 20, ports 22a-22d for respectively inserting a treatment tool to the inside of the retractor body 30 are provided. The converter 20 can be mounted on the opening end part 32 at an installation angle selectively chosen from a plurality of angles.
