FIELD: medicine. ^ SUBSTANCE: method relates to medicine, namely to cardio-vascular surgery and deals with estimation of state of myocardium by bioimpedancemetry when performing an operation on open heart. Method includes installation of sensor, which has not fewer than two electrodes, providing maximal coverage of examined part of myocardium. Measurement of electric impedance is carried out with at low from 5 to 20 kHz and high from 100 to 500 kHz frequencies. Measurements are carried out before beginning of artificial blood circulation, registering initial signal value, after clamping aorta and introduction of cardioplegic solution into coronary bed and after removal of clamp from aorta and recovery of coronary blood flow, registering current impedance values. If current impedance value, measured at low frequency, in process of introduction of cardioplegic solution is higher than initial value by 50% and more, myocardium perfusion is estimated as insufficient. If current impedance value, measured at high frequency, is by 25% and more lower than initial value, it is also considered to be evidence of insufficient myocardium protection. If after removal of clamp from aorta and blood flow recovery current impedance value, measured at high frequency, is by 85% and more lower than initial value, myocardium protection is estimated as inadequate. ^ EFFECT: method ensures adequate estimation of perfusion and myocardium protection and gives reasons for carrying out timely actions aimed at improvement said parameters during operation and at treatment of post-operation acute heart failure. ^ 1 ex |
