<P>PROBLEM TO BE SOLVED: To realize smooth medical services by allowing a plurality of medical staffs to open the same patient's electronic medical chart at the same time, and concurrently refer to, edit, create a variety of documents. <P>SOLUTION: A document creation device for electronic medical charts includes: a patient's file for recording electronic data of the patient; a data base for storing the patient's files; and a plurality of terminals configured to be accessible to the data base. The patient's file includes a plurality of record areas, and each the record area is independently accessible and editable.
