PURPOSE: An optical sensor module is provided to reduce the deviation of the coupling of the core of an optical waveguide unit and the optical device of the substrate unit. CONSTITUTION: An optical guide unit(W2) comprises an underclad layer(1), a core(2), an overclad layer, and a fitting unit. The core is formed in the surface of under clad layer. The over-clad layer coats the core. A pair of fitting units is formed in the over-clad layer by a suitable position to the light transmitting surface of a core. A substrate unit(E2) comprises a substrate, an optical device, and a fitted unit. The optical device is mounted in the certain position in the substrate. The fitted unit is formed in the suitable position substrate to the optical device.
