PURPOSE: A composition containing Bangpungtongseungsan or fermentation thereof is provided to prevent and/or treat osteoporosis. CONSTITUTION: A method for preparing a fermentation of Bangpungtongseungsan comprises: a step of adding water or organic solvent to crude drug mixture of Ledebouriellae Radix, Paeoniae Radix, Cnidii Rhizoma, Gardeniae Fructus, Forsythiae Fructus, Peppermint leaf, Zingiberis Rhizoma crudus, Ephedrae Herba, Rhei Rhizoma, Natrii sulfas, Platicodi Radix, Atractylodis Rhizoma alba, Glycyrrhizae radix, Scutellariae Radix, gypsum and talcum to prepare Bangpungtongseungsa; and a step of inoculating lactic acid bacteria to Bangpungtongseungsa and fermenting. The lactic acid bacteria include Lactobacillus, Bifidobacterium, Streptococcus, Leuconostoc, Pediococcus, or Lactococcus. A pharmaceutical composition for preventing or treating osteoporosis contains Bangpungtongseungsan or the fermentation as an active ingredient.
