<P>PROBLEM TO BE SOLVED: To provide a delivery slip which allows a delivery person to correctly and efficiently confirm instruction information from a customer in services given by a delivery company. <P>SOLUTION: A one-side opening type envelope part is formed by attaching an envelope lower sheet in which an adhesive layer is provided on its back surface and a release paper is releasably stuck to the adhesive layer, to two superposed envelope upper sheets, at three periphery portions. A slip to be pulled out is prepared between the envelope lower sheet and the envelope upper sheet. A plurality of notched parts are formed on the downside envelope upper sheet out of the two envelope upper sheets at two opposed periphery portions out of the three periphery portions at which the envelope upper sheet is superposed on the envelope lower sheet and attached, and an adhesive is applied to the three periphery portions on the envelope lower sheet. Half cuts are formed on the upside envelope upper sheet out of the two envelope upper sheets in such a state that each of them surrounds an area wider than an area superposed on the notched part on the downside envelope upper sheet, so as to attach the envelop upper sheet through the notched part in the area. Perforation for cutting off two envelope upper sheets is formed along the adhesive part of a base portion where at least the envelope part is formed on the two envelope upper sheets. <P>COPYRIGHT: (C)2012,JPO&INPIT |
