<P>PROBLEM TO BE SOLVED: To provide a delivery slip enabling a delivering person to correctly and efficiently confirm instruction information from a customer in services provided by an express company. <P>SOLUTION: An envelope lower sheet provided with an adhesive layer on its back surface and a peeling sheet attached to the adhesive layer and two envelope upper sheets laminated on the envelope lower sheet are bonded at three parts of the peripheral edge to form a one-side opening type envelope part. A pulling-out slip is provided between the envelope lower sheet and the envelope upper sheets in a state that it can be pulled out of the envelope part. In the laminated two envelope upper sheets, the lower-side envelope upper sheet is provided with a plurality of notches at two facing peripheral edge parts among three peripheral edge parts laminated and bonded to the envelope lower sheet, and an adhesive is applied to the three peripheral edge parts on the envelope lower sheet. In the laminated two envelope upper sheets, the upper-side envelope upper sheet is bonded to the adhesive via the notches. The two envelope upper sheets are formed with perforation for cutting out the two envelope upper sheets along the bonded part of a bottom part forming the envelope part. <P>COPYRIGHT: (C)2012,JPO&INPIT |
