Provided is a multilayer-recordable information recording medium with which highly-accurate tracking servo control is performed while employing the CLV mode, and with which a specific type of process, such as highly-accurate tilt detection, is performed while having enhanced information recording density. The disclosed CLV-mode information recording medium (11) includes: a guide layer (12) having tracks (TR) formed therein in advance; and a plurality of recording layers (13) laminated on the guide layer. The tracks include a plurality of guide regions (22) each having a physical structure that carries guide information for the purpose of guiding. The guide regions are arranged discretely along the track direction at intervals of less than or equal to a predetermined distance that has been defined in advance, and, over a plurality of said tracks that are adjacent to one another in the radial direction, the guide regions are misaligned from one another among the plurality of tracks. Further, the tracks include a plurality of signal detection regions (23) each having a predetermined pattern (23a) that is provided as a single block which extends over a plurality of track sections adjacent to one another in the radial direction, each signal detection region being arranged in a manner such that a specific type of pattern signal can be detected with at least the center track section among said plurality of track sections, which is located close to the center in the radial direction.
