PROBLEM TO BE SOLVED: To prevent leakage of personal information entered in a form or the like.

SOLUTION: A form entry stand 100 includes: a top plate 112 on which a form 300 is laid; a form DB 103 that holds class information for class recognition of the form 300 and attribute information of an entry area in the form 300; a photographing unit 101 that photographs the form 300 laid on the top plate 112; a form recognition section 102 that recognizes the class of the form 300 based on a photographed result by the photographing unit 101 and the class information held in the form DB 103; an input DB 110 that stores input information entered in the entry area in the form 300 by a user; an information control part 105 that draws the input information into the entry area in the form 300 based on a recognition result by the form recognition section 102, the input information stored in the input DB 110 and the attribute information held in the form DB 103; a drawing part 108; and a projection unit 109.
