<P>PROBLEM TO BE SOLVED: To provide a form creation device, a form creation method and program that can create an original form by customizing a form format easily. <P>SOLUTION: A form creation device 1 for creating an original form of an itemized statement includes: a template file acquisition section 16 for acquiring a template file corresponding to one page of the form; a data acquisition section 17 for acquiring data of the itemized statement; a definition table acquisition section 18 for acquiring definition tables, each defining the place where each item of the data is to be set in the template file; and an original form creation section 19 for creating the original form by executing form creation processing that takes the data into the template file according to the definition table.
