<p><P>PROBLEM TO BE SOLVED: To provide a terminal device that properly transmits an instruction item written on a form to each contractor as a recipient. <P>SOLUTION: The terminal device includes a receiving means, an instruction item extraction means, a recipient input receiving means, and an instruction item transmission means. The receiving means receives information written by an electronic pen in the entry form printed in a dot pattern. The instruction item extraction means extracts one or more instruction items based on the above information. The recipient input receiving means receives at least input on each of the recipient for the instruction items.
