Disclosed herein is the use of a skin contacting composition comprising i) a 5-HT3 receptor antagonist that is granisetron ii) ii) a permeation enhancer comprising 4% to 12% by weight of the skin-contacting composition, wherein the permeation enhancer consists essentially of a fatty acid ester of fatty acyl chain length C12-C18, or mixtures thereof; and iii) iii) an adhesive, in the manufacture of a medicament for treating actue, delayed or anticipatory emesis for a sustained period in an individual. Also disclosed is the composition for transdermal administration of an antiemetic comprising a skin-contacting composition as defined above, wherein, when applied to a portion of intact skin on an individual for 24 hours and then removed, the medicament provides the individual with a plasma concentration of the 5-HT3 receptor antagonist in a therapeutically effective range for period of time from an onset time to 12 hours or more after the medicament is removed.
