PROBLEM TO BE SOLVED: To provide a perforation forming apparatus having improved cutting easiness and preventing a formed image from being damaged by giving moderate visibility to the perforation. SOLUTION: The perforation forming apparatus including a sewing machine blade 311 having a plurality of blades formed on the circumference thereof, and a shaft member 365 that rotatably holds the sewing machine blade 311 is configured to bring the plurality of blades in contact with the desired portion of a workpiece, then, rotate the sewing machine blade 311 as a disk-like cutter to form the perforation with a plurality of continuous recessed portions, on the workpiece.
