Provided is a polylactic acid-based resin sheet which exhibits excellent impact resistance, transparency, slipperiness, and crease line formability and which is particularly suitable for formings. A polylactic acid-based resin sheet that comprises polylactic acid and rubber particles, characterized by satisfying the following requirements (1) to (3): (1) 0.01µm = |Ra1-Ra2| = 0.2µm (2) Ra1 = 0.3µm (3) Ra2 = 0.3µm [wherein Ra1 is the two-dimensional center line average roughness of one surface of the sheet; and Ra2 is the two-dimensional center line average roughness of the other surface thereof].
