PROBLEM TO BE SOLVED: To provide a particle beam irradiation apparatus capable of securing a prescribed flatness of the dose distribution over the whole area in the depth direction of an affected part without enlarging the scanning range or irradiation time of the particle beam. SOLUTION: The particle beam irradiation apparatus includes: a beam scanning part; a scattering body for enlarging the beam width of the particle beam to a prescribed beam width; a beam range enlarging device for dispersing and enlarging the in-vivo range of the particle beam according to the size of the affected part in the direction of progress of the beam; a compensation filter for matching the deepest in-vivo range of the particle beam to the contour shape of the affected part on the rear side; a collimator for shielding the beam irradiation to the outside of the contour of the affected part on a plane orthogonally crossing the direction of progress of the beam; and a flatness monitor for measuring the flatness of the dose distribution on the plane orthogonally crossing the direction of progress of the beam. The respective components are disposed in such order as starting from the closest to the affected part; the compensation filter, the collimator, the beam range enlarging device, the flatness monitor, the scattering body and the beam scanning part are disposed in this order.
