PROBLEM TO BE SOLVED: To provide an image reader for properly reading a document image, and to provide an image forming apparatus. SOLUTION: An image of a document is read by a contact type image sensor 16 positioned below a conveyed image read position 25. The document whose image is read is guided by a downstream side guide member 19 provided in the document conveying direction on the downstream side of the conveyed image read position 25. A black print area 28 is provided on a transparent film member 18 which is provided so that the document conveying direction upstream side end is positioned closer to the document conveying direction upstream side than the conveyed image read position 25, and the document conveying direction downstream side end is positioned on the downstream side guide member. The transparent film member 18 guides the document conveyed to the conveyed image read position 25 to the downstream side guide member 19.
