The instrument has a hollow gripping handle including a proximal end and a distal end provided with a rotating head for forming and placing a tie. The head has a hollow movable jaw (51) articulated with respect to a hollow fixed jaw (50) along a geometrical axis. A wire cutting mechanism is carried by a movable spool (8) in the fixed jaw. The spool is displaced between a high limit stop position i.e. rest position, and a low limit stop position i.e. withdrawal position, in which a semi-rigid wire is released for being exposed to a gripping action of a wire gripping unit of the movable jaw.
