PROBLEM TO BE SOLVED: To easily form a lenticular print sheet by which a visual effect that an image looks as if it moves is obtained to order, and to discriminate it in a required state and convey and send it out. SOLUTION: The lenticular print sheet 1 is constituted of a print sheet 11 on which a composite image 12, at least a part of which can be varied on the basis of input image information, and information display parts 17 and 18 for displaying information of the composite image are printed, and a lenticular lens 15 stuck to a printing area 13 of the composite image. The lenticular print sheet is constituted such that a lenticular lens sticking part to the print sheet is formed by being separated from a punched frame sheet part by being punched and cut in prescribed size.
