PROBLEM TO BE SOLVED: To provide a failure determination device for a fixing device that can predict the failure or specify the failure position in the fixing device, in which the linear velocity of a second fixing roller is differentiated from that of a second pressure roller to polish the surface of the second fixing roller and thereby it is possible to repair the degraded surface of the second fixing roller. SOLUTION: The failure determination device includes: a first feature amount extracting means to extract a first feature amount representing a time interval between each implementation of surface repair; a second feature amount extracting means to measure the surface state of the second fixing roller 3a before and after the implementation of the surface repair and extract a second feature amount representing a change in the surface state of the second fixing roller; a third feature amount extracting means to extract a third feature amount representing a change in a current value of a third drive motor m3 (rotation drive motor); and a determination means to determine to predict or specify the failure by performing weighted majority rule determination for the result of preliminary determination, by performing preliminary determination by means of stamp weak determination device respectively corresponding to the first to third feature amounts.
