PROBLEM TO BE SOLVED: To provide an image forming body or an individual authentication medium excelling in forgery-alteration preventing performance and having images facilitating authenticity determination. SOLUTION: An image forming body manufacturing apparatus includes at least a supply mechanism for supplying a film to one surface of a base material; a storage mechanism for storing individual authentication information in an individual authentication information storage means; and a display mechanism which has a light transmitting liquid crystal panel arranged parallel with and facing a surface, having a second visible light transmitting layer, of a film and having a plurality of pixels with a light transmitting function and a light shielding function, arranged on a matrix, and which displays an image pattern on the light transmitting liquid crystal panel based on the image information to form a light shielding mask.
