PROBLEM TO BE SOLVED: To provide a suitable image for diagnosis by an X-ray image diagnostic device when a region including a part like lower limbs, etc. where direct radiation is detected, is fluoroscoped. SOLUTION: The X-ray image diagnostic device 10 includes: an X-ray irradiator to irradiate X-ray to a patient's predetermined region, an image receiver to receive the X-ray; a top plate to place the patient on, an image processing circuit 52 to generate an image based on data obtained by the image receiver under the X-ray conditions for every position of the top-plate moving direction which is the direction in which the top plate moves along its long axis; a part 611 to set up a plurality of regions of interest and a part 612 to divide the region of interest into several parts on the image; a profile generation part 613 and a correction coefficient calculation part 614 to respectively calculate the correction coefficient for each of the regions of interest constituting the plurality of regions of interest; an image level calculation part 615 to calculate the image level of the image by respectively executing corrections for brightness value groups in the respective regions of interest using the correction coefficient corresponding to the respective regions of interest; and a see-through condition determination part 616 and a see-through execution part 617 to obtain X-ray conditions based on the image level, and control the X-ray irradiator by the X-ray conditions.
