PROBLEM TO BE SOLVED: To provide an image reader capable of preventing not only the sticking of foreign matters stuck to an original but also the distortion of read images by a simple configuration, and an image forming apparatus including the same. SOLUTION: The image reader 6 includes a second cleaning member 73 which is adjacent to the upstream side of the first image read region R1 of automatic read glass 25a relative to the original conveying direction of an original P. The second cleaning member 73 is formed in such a manner that, by forming an inclination part 73aa at both ends in the original width direction of a downstream side end 73a in the original conveying direction, the width of the downstream side end 73a is narrowed toward the downstream side in the original conveying direction. When reading the original P, the rear end Pa of the original P which passes through the second cleaning member 73 is successively abutted on the automatic read glass 25a from both ends in the original width direction toward the center.
