HEALTHCARE SYSTEM AND METHOD FOR PATIENTS WITH CHRONIC DISEASES

PURPOSE: A healthcare system and method for patients with chronic diseases are provided to continuously supply nutrition treatment and nutrition management program proper to the chronic ill by reflecting the health state and nutrition state of the chronic patients including the patients with metabolic syndrome. CONSTITUTION: An input unit(10) inputs the information for the health management of the chronic ill, and a control unit(20) sets up the nutrition management target proper to the health and nutrition state of the chronic ill. The control unit calculates the nutrition treatment for achieving the setup nutrition management object, and calculates the nutrition management program in order for the chronic ill to manage self-management according to the calculated nutrition treatment. An output unit(40) outputs the information necessary to the calculated nutrition treatment and the set nutrition management object.
