PROBLEM TO BE SOLVED: To provide an image forming apparatus which can easily recognize at least a copied document when the document including a code symbol is copied, and an image processing program. SOLUTION: An extraction part 121 extracts a code symbol part from image data, and a color replacement part 124 replaces at least a part of the code symbol with a predetermined color. In addition, a deletion part 122 deletes the code symbol part from the image data, and a color conversion part 104 converts the image data except the code symbol into image data to be shown in a color space to be utilized in image formation. Then, a screen generation part 105 synthesizes the image data converted by the color conversion part 104 with the image data of the code symbol part of which at least a part of the color is replaced by the color replacement part 124 to generate a screen to be utilized in the image formation.
