PROBLEM TO BE SOLVED: To obtain an image-formed body or a personal identification medium which has excellent performance of preventing forgery and interpolation and an image easy to determine whether it is true or not. SOLUTION: A second visible light beam transmissive layer photocuring step is made to include such pattern exposure that a light shielding mask is formed by arranging a light transmissive part and a light shielding part to display a desired individual information pattern, the light shielding mask is placed on a transparent base material, the resulting transparent base material is irradiated with light through the light shielding mask to partially cure a second visible light beam transmissive layer, so that the image-formed body, on which a rugged part for diffracting or scattering external light and a flat part are arranged selectively and which has personal identification information, is produced by an on-demand system.
