PROBLEM TO BE SOLVED: To provide a resin form which facilitates the grasp of a usage state. SOLUTION: This resin form includes: a resin form body which comprises a contact surface coming into contact with placed concrete and a noncontact surface positioned in parallel with the contact surface and on the other side of the contact surface; a barcode carrier 10 which is fixed to the noncontact surface, which is made of a transparent or translucent material, and which comprises a parallel surface 2 parallel to the noncontact surface and a plurality of vertical surfaces 3-6 perpendicular to the noncontact surface; a colored liquid which is stored in the bottom of the barcode carrier; and a plurality of barcode labels 7 which are stuck on the parallel surface and the plurality of vertical surfaces, respectively. A barcode with a missing portion 8, which becomes valid only when a barcode label-stuck surface is directed vertically downward and colored with the colored liquid and which becomes invalid in the other cases, is indicated on each of the plurality of barcode labels 7.
