<P>PROBLEM TO BE SOLVED: To provide a medical instrument and a medical kit easily communicating inner cavities of two lumen organs with each other. <P>SOLUTION: The medical device includes a guide system, easily communicating two lumen organs with each other without operation to insert a wire while keeping the tip of a puncture needle and the tip of a catheter inside the lumen organs, a delivery sheath shaped cylindrical, a dilator, of which tip is formed as a tapered reduced diameter part, and inserted into the delivery sheath to protrude the reduced diameter part from the tip of the delivery sheath, and the pulling wire to pull the dilator in the protruding direction of the reduced diameter part.
