Provided is a lens driving device wherein a reduction in perpendicularity of a guide member in relation to the base is suppressed. Also provided is a camera module equipped with said device. The lens driving device is provided with a base (30) having a base section (31), a holder which holds the lens and can move in the direction of the optical axis of the lens relative to the base (30), and shafts which are disposed on the base (30) so as to be oriented in the direction of the optical axis and guide the movement of the holder in said direction. The base (30) is provided with a support column-side shaft receiving section (36) which receives the top of the shafts in the direction of the optical axis, and a base-side shaft receiving section (37) which receives the bottom of the shafts in the direction of the optical axis. The support column-side shaft receiving section (36) and the base-side shaft receiving section (37) are configured as a single member.
