PURPOSE: A form for a beam structure is provided to improve the working environment and stability of construction by structurally reinforcing strength. CONSTITUTION: A form(600) for a beam structure comprises a lower plate(100), a first side plate(200), a second side plate(300), and multiple side support members(400). The lower plate is formed in a rectangular shape with a length corresponding with the length of a beam structure. The first side plate is vertically positioned in one side of the lower plate and has first beads in a width direction. The second side plate is vertically positioned in the other side of the lower plate and has second beads(310) in a width direction. The side support members are horizontally arranged in a longitudinal direction of the first and second beads to be separated from each other and are coupled to the first and second beads.
