PROBLEM TO BE SOLVED: To suppress a situation that movement of an actuator is impeded and a situation that the wiring of a flat flexible board connected to the actuator is short-circuited or damaged. SOLUTION: A plurality of lands 58 are formed on base material 51. A first cover coat 65 is formed by screen-printing first insulating epoxy resin and also perfectly curing it in an area except the land 58 related to a land area 60 where the land 58 is formed and the periphery thereof. A second cover coat 66 is formed by screen-printing second insulating epoxy resin and also semi-curing it in respective holes 65a of the first cover coat 65. An actuator unit 21 and a COF 50 are arranged so that an individual bump 136 pierces through the second cover coat 66 in a semi-cured state to abut on the opposed land 58.
