An implantable access port and delivery tool system includes an implantable access port with a base 510 and a delivery tool to facilitate implantation of the access port into soft tissue. The base of the access port includes fasteners 501 pivotally connected to the base and movable between an undeployed and a deployed position, and a base mounted rotatable element 525 that is rotatable to deploy the fasteners from the undeployed position. The delivery tool includes a delivery tool rotatable element configured to removably and rotatably engage the base mounted rotatable element, and an activation assembly having a transmission element to rotate the delivery tool rotatable element.
