PROBLEM TO BE SOLVED: To provide a document management system for converting documents into electronic data to register them in a database without depending on the visual confirmation of people.

SOLUTION: A doctor receives an ambulatory essential form 110 submitted by a patient and a chart 100 for entry or the other medical treatment record printed by a terminal 10 for printing documents, writes results of medical examination or tests on the chart 100 for entry or the medical treatment record, and superposes the first form as the ambulatory essential form 110 on the second and subsequent forms as the entered chart 100 for entry or the other medical treatment record to create a registration document bunch 120. In addition, the doctor causes a scanner 40 to read the registration document bunch 120 in a batch by operating a terminal 30 for registering documents in such a manner that image data or text data of the chart 100 for entry or another medical treatment record can be registered in the database stored in the document management server 50.
