<p><P>PROBLEM TO BE SOLVED: To provide a consent form management system capable of accurately managing a consent form corresponding to medical service in order to smoothly provide the medical service necessary for a patient from a medical staff. <P>SOLUTION: The consent form management system, for managing a consent form expressing patient's consent to the medical service provided from a medical staff, includes a consent form database for storing document data of a plurality of sorts of consent forms to be used for the medical service. When an input concerned with the medical service to be provided to the patient is accepted by the medical staff, the document data of one or more consent forms to be acquired to provide the medical service are acquired from the document data stored in the consent form database based on the input.
