PROBLEM TO BE SOLVED: To provide an image reader capable of suitably performing shading correction even when a light source unit runs obliquely in a sub-scanning direction and to provide an image forming apparatus including the image reader. SOLUTION: A document 96 is set in a predetermined reading target area on a first document rest 21 in the image reader 20. A first white reference member 41 and a second white reference member 42 are arranged so as to sandwich the reading target area between the members 41, 42 in the sub-scanning direction 95. A control part 400 compares first white reference data 51 with second white reference data 52 to acquire a deviation M1 in a main scanning direction 94 of a light source unit 23 when the light source unit 23 is moved from a first white reference data reading position 43 up to a second white reference data reading position 44 in the sub-scanning direction 95 and to perform shading correction of image data of the document 96 based on the deviation M1.
