PROBLEM TO BE SOLVED: To provide a check list preparation apparatus, a check list preparation method and a check list preparation program, for quickly preparing a check list optimum to a machine to be evaluated without depending on an evaluator. SOLUTION: The check list preparation apparatus includes: a component information acquisition means 11 for acquiring component information of the target machine 1; a fundamental motion evaluation item storage means 13 for storing the component information of the machine and evaluation items for checking the fundamental motion of components in association with each other; and a fault information storage means 14 for storing the component information of the machine and fault information concerned with the components in association with each other. Further, the check list preparation apparatus includes a check list preparation means 12 for extracting evaluation items corresponding to the component information of the target machine 1 from the fundamental motion evaluation item storage means 13 and the fault information storage means 14, and for creating a check list to be used for evaluating the target machine.
