PURPOSE: A home-based remote home care service system and a method thereof for a patient with a chronic disease is provided to use wearable biometric measurement device of chronic disease and measure a bio-signal of the patient with the chronic disease, thereby measuring the bio-signal of the patient without sending a home care nurse. CONSTITUTION: A patient device includes next. A wearable biometric signal measurement unit(110) measures bio of a patient by having more than one sensor. A communication unit(130) transmits bio-measurement data and photograph data to a data preservation server(200). The data storing server classifies and stores the bio-measurement data and photograph data according to each patient.
