PROBLEM TO BE SOLVED: To provide a delivery slip including fewest pages possible of a paste slip necessary for a shipping process, a shipping slip, and a delivery document, which can be manufactured at a relatively low cost, and furthermore, which enables delivering a delivery item with the delivery document pasted thereon together with the paste slip and delivery slip. SOLUTION: The slip base material, which keeps a paste portion to be pasted on a parcel and a delivery document portion for submission to the receiver of the parcel, connected to each other on the boundary of the cut-off portion. The backside of the paste portion to be pasted on a parcel of the slip base material has an adhesive layer formed thereon and, release paper is pasted in a releasable manner on the backside of the adhesive layer. The release paper includes cuts so that when the delivery slip is pasted on the object, it can be released from the adhesive layer, separating itself from the other portion, leaving an area for inserting and enclosing the delivery document portion of the delivery slip base paper between the release paper and the object.
