<P>PROBLEM TO BE SOLVED: To provide a notebook for recording artificial tooth treatment for accurately carrying out medical action by improving mutual understanding between a patient who requires new production of artificial teeth and treatment such as repair, and a medical worker such as a dentist. Ž<P>SOLUTION: The patient writes down occasional demands and symptoms that occur in artificial tooth treatment, beforehand in this notebook for recording artificial tooth treatment, and brings the notebook on the medical examination day. Three-dimensional models of jumping-out artificial teeth of an artificial tooth board and a drawing of the artificial teeth in an artificial tooth treatment record facilitate indication and entry of complicated portions of the artificial teeth that cannot be easily expressed in words. The dentist carefully reads information in the oral cavity written in the notebook for recording artificial tooth treatment including the artificial tooth board, to shorten a consultation time with the patient, and the treatment action and treatment time desired by both sides can be acquired.
