PROBLEM TO BE SOLVED: To provide a form rolling tool and a spline shaft, capable of enhancing the form rolling accuracy of the spline shaft having an irregular portion for phase focusing. SOLUTION: A first raised tooth 13 has a plurality of first flank faces 13b, and a plurality of first raised flank faces 13c arranged on the first flank face 13a side with respect to the first flank face 13b, and the first raised flank faces 13c and the first flank faces 13b are alternately arranged in the tooth trace direction. Thus, by arranging a fore end of a material to be form-rolled at a different position in the tooth trace direction to perform the form rolling by a thread flat rolling die 1, a spline shaft of the spline of different length can be form-rolled. Further, when the spline is long in length, the portion of the spline shaft to be form-rolled by the first raised flank face 13c can be reduced by the amount of arrangement of the flank face 13b, resulting in improvement in form rolling accuracy of the spline shaft.
