<p><P>PROBLEM TO BE SOLVED: To provide a form design device for easily designing a form, improving the recognition rate of an electronic pen by printing a dot pattern also in a peripheral area with a hiding ink, while solving design problems. <P>SOLUTION: A designer uses the form design device and specifies positions of items constituting the form by GUI or the like in designing. The form design device acquires information related to the positions or sizes of items specified by the designer as item position information, and prepares dot plates to be arranged in an item area and a peripheral area based on the item position information. The form design device forms mask plates to be further disposed on the dot plates in the peripheral area based on the acquired item position information.
