PROBLEM TO BE SOLVED: To provide a sheet for a card sending mount which is suitable for sending an identification number table indicating an identification number by a random number table together with a card, the card sending mount using the sheet, and a method for manufacturing the card sending mount. SOLUTION: The card sending mount 11 includes a card receiver's address information printing part, a card holding member forming part, and an identification number table printing part on the same surface side of three pieces of paper 4, 5, and 6 which are connected through perforations 2 and 3 for folding. Card receiver's address information 12 is printed in the card receiver's address information part, a card holding member for holding the card 13 detachably is formed in the card holding member forming part to hold the card 13, and an identification number table 14 in which an identification number for identifying a formal card owner by an internet bank offering card utilization object service is indicated by the random number table is printed in the identification number table printing part.
