The upper form panel (2) has a foot cylinder (26) i.e. screw cylinder, with a movable foot (38) supported on ground. The cylinder has a centering unit centering the upper panel with respect to a lower form panel (2A), and a locking bore (82) locking the upper panel against the lower panel. The foot is movable in translation with respect to a body of the upper panel along a translation axis (A). The centering unit has a cylindrical outer centering surface (80) co-operating with a centering bore (91) of the lower panel and an additional centering bore (42) co-operating with the surface.
