<P>PROBLEM TO BE SOLVED: To provide a compact form processing device capable of reducing the number of units of a conveyance system, controlling the conveyance system easily, and automating part of recycling process for a form. Ž<P>SOLUTION: This form processing device 100 performs the processings for reutilizing the used form 10 used to place and mold concrete and mortar, and the used forms 10 are sequentially carried into the conveyance system 120 by a carrying-in part 110. The conveyance system 120 includes a rail 121 provided with a pair of right and left driving conveyance chains 122 and a roller 123 to convey the used forms 10 sequentially. A processing part 130 provided with a processing mechanism for the form 10 is provided in the conveyance process of the conveyance system 120 to perform various kinds of processings. A carrying-out part 140 receives the processed form 10 and carries it out. The carrying-in part 110 includes a lift device moving up and down just below the conveyance system 120 to carry the form 10 into the conveyance system 120 from below the conveyance system 120.
