PROBLEM TO BE SOLVED: To provide an image forming apparatus enabling a user to determine whether to carry out print output of a sample image when preset sample paper delivery conditions are established. SOLUTION: The image forming apparatus 1000 includes an operation unit 40 setting the sample paper delivery conditions by user operation; a counting part 38 for measuring a decision object value for determining whether the sample paper delivery conditions are fulfilled; a temperature sensor 55; and a humidity sensor 56. An overall control unit 31 carries out a decision program 35c and a notification program 35d. When the sample paper delivery conditions are determined to be fulfilled, an announcement display screen is thereby displayed on a display 42, and an announcement (notification) is made to a user to the effect of urging the execution of sample paper delivery.
