The invention relates to a device for fixing screws in osteoporotic bones, formed by biocompatible fixing elements and an instrument for positioning same. The instrument consists of two pieces, namely a male piece (A) and a female piece (B), which can perform a relative linear sliding movement. The caudal end of each piece is provided with a pivoting clamp (Pa, Pb) forming an inverted U therebetween. The biocompatible element is positioned in the clamp of the male part and is traversed by a screw that is inserted through interchangeable guides (g) positioned in the other clamp, said biocompatible element serving as a nut for locking and fixing the screw. Both of said pieces are provided with handles (m) which can be used to move the clamps closer together. The device can be opened under the action of a compression spring (R) using the thumb to release the catch inserted into the upper saw-tooth face of piece A. The opening of the device is restricted by a closure element (C).
